Center any time, day or night! The Assessment Referral Center is
open 24-hours a day, 7 days a week to take your phone calls, do
assessments and facilitate admissions.
Will My Insurance Company Pay
For The Services?
Rockford Center has contracts with most major insurance
companies and managed care organizations. If you are uncertain whether
your insurance will cover the cost of treatment, you may contact

What Should I Bring With Me
to Rockford Center?
In the event that inpatient admission is necessary,
you may want to bring the following items with you to Rockford Center:
- Current identification and insurance cards
- Overnight personal care items including shampoo/conditioner,
toothbrush, toothpaste, deodorant, hair comb & brush, body
lotion
- All current medications
- Casual clothing
- Comfortable shoes without laces
- Small amount of money for small purchases
(i.e. cigarettes, snacks, stamps and personal items)
Do Not Bring:
- Televisions, radios, CD players, or electronic
games
- Jewelry
- Any substances that contain alcohol, drugs,
or other substances that could be abused
- Tight fitting or short cut clothing, shirts
with inappropriate slogans or revealing slumber wear
- Food or beverage of any kind
- Cellular Phones
- Shoes with laces
- Purses, or handbags of any kind
- Medications (illegal, prescription or over
the counter)
- Glass items
- Lighters or matches
- Plastic bags, or hangers
- Scissors, knives, razors, sharp objects or
weapons of any kind
- Metal files, picks, nail clippers
- Safety pins or straight pins
- Electronic devices, including cell phones,
pagers, ipods, cameras, computers, tape recorders, radios,
hairdryers, curling/straightening irons, CDs, etc.
- Belts, suspenders, scarves, ties,
shoelaces or strings in any clothing item, hats
- Blankets, pillows, stuffed animals (child
unit only), towels, washcloths
- Aerosol sprays, spray bottles or canisters
(including perfume or body sprays)
- Makeup (exception: adults may have
powder foundation without mirror and lipstick)
- Jewelry (exception: wedding rings,
religious items, or post earrings)
- Footwear: heeled shoes or boots of any
kind (exception: adults may wear slippers or flat laceless
shoes/sneakers; adolescents may only have slippers)
- Underwire from bra
- Thong underwear (exception: adults
may have at discretion of staff)
- Any items considered to be potentially
dangerous by staff
- ANY ITEMS CONSIDERED TO BE UNSAFE OR
VALUABLE ARE TO BE SENT HOME WITH FAMILY MEMBERS.
- ROCKFORD CENTER WILL ONLY ASSUME
RESPONSIBILITY FOR THOSE ITEMS DEPOSITED IN THE SAFE.

Will My Family Be Involved in Treatment?
Family education and family therapy are essential
components of the program and recovery. Prospective clients and
their families are informed of the expectations of family
participation prior to admission to the program.
Do I Have To Participate In
My Treatment Plan?
All individuals are expected to actively participate
in their own treatment. Your involvement and willingness to work
with the staff of Rockford Center are crucial elements of treatment.
Rockford Center staff also encourages the involvement of family
members in the treatment process.
