If I Have A Problem On A Weekend Or Evening, How Do
I Contact Rockford Center?
The Assessment Referral Center is open 24-hours a day, 7 days a week to
answer phone calls, handle assessments and facilitate admissions.
Will My Insurance Company Pay For The Services?
Rockford Center has contracts with most major insurance companies and managed
care organizations. If you are uncertain whether your insurance will cover the
cost of treatment, you may contact Rockford Center at 302.996.5480 or

What Should I Bring With Me to Rockford Center?
In the event that inpatient admission is necessary, you may want to bring the
following items with you to Rockford Center:

Current identification and insurance cards

Overnight personal care items including shampoo/conditioner, toothbrush, toothpaste, deodorant, hair comb & brush, body lotion

All current medications

Casual clothing

Comfortable shoes without laces

Will My Family Be Involved in Treatment?
Family education and family therapy are essential components of the program and
recovery. Prospective clients and their families are informed of the
expectations of family participation prior to admission to the program.
Do I Have To Participate In My Treatment Plan?
All individuals are expected to actively participate in their own treatment.
Your involvement and willingness to work with the staff of Rockford Center are
crucial elements of treatment. Rockford Center staff also encourages the
involvement of family members in the treatment process.
What Should I Expect Upon Arrival at Rockford Center?

Client or parent/guardian complete registration form with emergency and outpatient provider contact information and insurance information
-
Level of Care assessment performed by master's level counselor to determine recommendation of care (takes minimum 45 minutes)

For inpatient recommendations: Admission paperwork and orders completed (takes minimum 30 minutes). After admission to unit, a nursing assessment is completed with a focus on medical history (takes minimum 30 minutes). Non-invasive client search and belongings search completed by staff to ensure safety.

For partial day recommendations: Client sent home and staff to contact insurance company for authorization next business day. Client will begin program 1-2 days after initial assessment. *Transportation may be available*

For all other recommendations: Client given plan of care form that lists 3 appropriate service providers in their home area to contact.
What Should I Expect Within the First 24-48 Hours?

Referral source and outpatient providers notified of admissions (only with signed release of information)

Am I Able to Receive Visitors During My Stay at Rockford?
Visitors are welcome during your stay, according to the following guidelines:

All visitors will be required to sign in for visits as well as show photo identification. Visitors will be taken from lobby in 15 minute increments. To ensure a full visit, please arrive 15 minutes prior to visitation to sign in and receive a guest badge. Children 12 and under will not be permitted to visit.

Adult Units: Monday & Wednesday 7:00 p.m. - 8:00 p.m. Sunday 1:30 p.m. - 2:30 p.m. Progressions (Geriatric) Wednesday 12:00 p.m. - 1:30 p.m. Saturday 11:00 a.m. - 12:00 p.m. Daily 5:30 p.m. - 7:30 p.m. Adolescent Unit Tuesday & Thursday 7:00 p.m. - 8:00 p.m. Saturday 1:30 p.m. - 2:30 p.m. Child Unit Monday - Friday 5:30 p.m. - 6:30 p.m. Weekends 12:30 p.m. - 1:30 p.m.
