Rockford Partial Hospitalization Program Brochure – Patients transitioning from inpatient psychiatric care or who require more intensive services than those offered in traditional outpatient therapy will benefit from Rockford Center’s Partial Hospital Program. What to Expect
During an initial assessment, our primary goal is to evaluate individual needs and make appropriate clinical recommendations. Assessments include:
• Evaluation of the presenting problem
• Mental health and substance abuse history
• Current mental status examination
• Safety assessment
• Level of care determination
• Financial considerations
• Plan of action, including referrals, for ongoing support
